    (EVERY parent/guardian MUST complete this form.)

    • Please use appropriate language.

    • Please respect others personal space- Keep hands and feet to yourself.

    • Please cooperate when asked to do a job or take part in a group activity.

    • Please respond when spoken to by staff appropriately. This is a two- way system in which staff should respect the participant as well.

    • Help clean the area at the end of the activity.

    • Ask to go to the bathroom (Parks are big and we need to know where everyone is).

    • Please ask to play with others personal belongings with anyone else's-toys, sports equipment, etc.

    • Do not take anything home that is NOT yours.

    • Listen to and follow directions at all times.

    • Respect self, staff, and others.

    • If you have any questions, please ask someone for help.

    BEHAVORIAL CONSEQUENCES

    Problem Disipline Procedure
    Swearing 1st time – Explain
    Insubordinate behavior 2nd time - Time out & Explain
    Unsportsmanlike behavior 3rd time – Repeat Time out & Explain
    Talking back to staff 4th time – We’ll talk to parents
    Taking others items 5th time – Dismissal
       
    Fighting & Stealing 1st Step - Time Out and talk to parents
      2nd Step- Repeat Step 1 and Final warning
      3rd Step - Dismissal
